Job Description:

Position Summary/Department Summary: 40-hour 100% onsite social work position to provide a range of clinical social work services and case management to patients in the lung, intestine and liver transplant program. This social worker will care for the organ group population in all phases of transplant (evaluation, transplant admission and follow up).
