: 13 Weeks Shift : Days (5*8) Blended Pay: $74-$84/hr Start Date:

Nurse Navigator plays a critical role in guiding patients through
their healthcare journey, ensuring they receive the appropriate
care and support throughout their treatment process. This position
requires a compassionate and organized individual who can
effectively communicate with patients, families, and healthcare
providers. The Nurse Navigator will coordinate patient care,
facilitate access to services, and provide education on treatment
options, particularly in areas such as ophthalmology and acute
care. Responsibilities Serve as the primary point of contact for
patients, addressing their questions and concerns throughout the
treatment process. Coordinate care plans with multidisciplinary
teams to ensure comprehensive patient management. Assist in
medication administration and educate patients about their
prescribed treatments. Conduct specimen collection processing and
perform venipuncture as needed. Utilize Epic or similar electronic
health record systems to document patient interactions and track
progress. Provide support in acute care settings, including trauma
medicine and airway management when necessary. Facilitate referrals
to specialists and follow up on appointments to ensure continuity
of care. Educate patients about procedures related to ambulatory
surgery centers and assist with pre-operative preparations.
Maintain accurate records of patient interactions, care plans, and
outcomes in compliance with regulatory standards. Facility
Requirements: Minimum 2 years of experience required. Active BLS,
