Pay Information $2,888 per week About The Position Job Title: Home Health RN Profession: RN Home Health Specialty: RN Home Health Duration: 13 weeks Shift: 08:00 AM - 04:30 PM Hours per Shift: 8 Experience: Minimum 1 year License: Valid RN License Certifications: Certified Home Health experience, Case Management experience Must-Have: - 1 year Certified Home Care experience. - 1 year Case Management Experience. - Ability to travel to cases as assigned. - Weekend rotations required once every 6 weeks. Description: As a Home Health RN, you will be responsible for assessing patient conditions. You will implement care plans for patients in your care. Providing ongoing education and support to patients and their families is essential. Patient care must be delivered within organizational philosophy, policies, and standards of community health practice. You will be required to perform admission processes and case management responsibilities. This is in accordance with established policies and standards of practice.
